Build a survey in the block editor, drop it on a page, and watch the answers arrive. Every response is collected in your own dashboard — no third-party service, no account, and nothing leaves your site. Surveys can live straight on a page, or be saved once under Survey Forms → Surveys and printed anywhere with a shortcode. Edit it in one place and every copy follows. Response Dashboard: Read, search, sort, page through, edit and delete submissions from the WordPress admin. Nothing is sent to an external service. Works With Page Caching: Submissions are not tied to an expiring nonce, so visitors on a cached page can still answer. Flooding is limited per IP address instead. Reusable Surveys & Responses Survey Forms → Surveys is for surveys you want in more than one place. Each entry holds a single survey and gets its own shortcode, shown in the ShortCode column — click it to copy, then paste it into any page, post or widget, or call do_shortcode() from a template. Because every copy points at the same survey, editing it once updates all of them, and every copy writes to the same set of responses. Survey Forms → Survey List shows what people have answered: pick a survey, search across the answers, sort by any question, page through, correct a response in place, and delete one or several at a time. A Refresh button pulls in anything that arrived while the screen was open. Dropping the Survey Form Block straight onto a page still works exactly as before — the Surveys screen is an addition, not a replacement. How to use Install and activate the plugin. Add the Survey Form Block from the “Widgets” block category in the editor. Build your questions and style them from the right-hand sidebar — the General, Design and Style tabs. Publish the page. Answers appear under Survey Forms → Survey List. To use the same survey in several places, add it under Survey Forms → Surveys instead and paste its shortcode wherever you need it.
